    Welcome to our family! Your health. What could be more valuable? At the E.C. Tyree Health & Dental Clinic, putting patients first is, and always has been, our mission for over 23 years. We believe everyone should have access to quality health care that not only give solutions but walks with our clients every step of the way.

    The Clinic offers a wide range of services to fulfill the needs of your entire family. This page was created to give you some things to help you prepare for your first visit.

First visit tips:

    1. Arrive 10 to 15 minutes early to complete necessary paperwork. You may download and fill out the paperwork on this page prior to your visit.
    2. Bring a current copy of your insurance or medical card.
    3. Self-pay clients must bring in proof of income. A current paycheck stub or official statement of assistance from a state assistance program.
    4. All minors under 18 must be accompanied by an adult.
    5. Be prepared to pay insurance co-pays at the time of service.
    6. Bring a copy of any forms that need to be completed.
    7. Have a list of all current medications.
